**In the event that you need to reschedule or cancel your appointment, please call, text or email AT LEAST 24 hours before your scheduled appointment. Late notice cancellations (within 24 hours of your appointment) or no-shows are subject to a fee of 50% of the total cost of your appointment as scheduled. Payment for cancellations and no-shows are required within 48 hours of your appointment. Non-payment will result in cancellation of any future appointments and no further appointments permitted. **For first time clients: please have your Health History form filled out and/or confirm your appointment at least 24 hours before your appointment. If you have any issues completing your Health History or confirming your appointment, please contact me! If you do not complete your form and/or confirm your appointment and I don't hear from you, your appointment will be CANCELLED. Thank you!**
